Gift Aid declaration template (HMRC-compliant)

How to use this: use HMRC’s model wording without changing the mandatory parts. You may add your own reference numbers or branding around it. Give donors the declaration to complete, keep the record, and only claim Gift Aid where you hold a valid declaration. The wording below follows HMRC’s model declaration.

Gift Aid lets your charity reclaim 25p for every £1 donated by an eligible UK taxpayer who has made a valid declaration, at no extra cost to the donor (Source: HMRC / gov.uk, accessed 10 July 2026).

Single donation declaration

If you pay Income Tax at the higher or additional rate and want to receive the
additional tax relief due to you, include all your Gift Aid donations on your
Self Assessment tax return or ask HMRC to adjust your tax code.

Notes: the parts you must not alter are the charity name, the “I want to Gift Aid…” statement, the taxpayer responsibility statement, and the donor’s full name and home address fields. Keep each declaration for at least six years from the end of the accounting period (enduring declarations permanently). For a verbal declaration, send the donor written confirmation and tell them they can cancel within 30 days.
